The Family Advantage Federal Credit Union is pleased to offer a unique LifeFlight insurance policy.
What Is LifeFlight Coverage?
A Reimbursement Program to cover Life Flight and/or Ground Ambulance transportation expenses due to an emergency.
LifeFlight Coverage Points:
Covered Member: Any member and their dependents (if family coverage is selected). Dependents are covered over the age of 18 as long as the dependent is enrolled in school full-time. In the event of an accident, the student must be enrolled full-time.
Benefit: The maximum benefit per occurrence is $15,000, but there is no Annual Aggregate limit.
Coverage: World-Wide—the coverage goes with you in the event you relocate or when traveling.
Occurrence: A single recorded incident involving a covered member.
Deductible: There are no deductibles.
Expiration: There is no renewal date. Your coverage continues as long as your membership is in good standing. It is the member’s responsibility to ensure that enough funds are on deposit to cover your monthly membership fee.
Other Insurance: In the event other valid and collectible insurance is available, this policy will become secondary or excess. A.I.M. reserves the right to investigate claims to determine if there is other primary coverage available.
Steps to Properly File a Claim:
Do not send any paperwork to the Credit Union as this will only delay processing.
Do not present your LifeFlight or Credit Union membership card to the hospital for billing. Your primary health coverage insurance card should be presented. You will receive a statement from your Primary Provider showing what has and has not been paid. Send that "Explanation of Benefits" directly to:
